山东省淄博市“09.30”模板坍塌事故一、事故简介 2006年9月3O日，淄博市某碳酸钙厂二次混料室工程在施工过程中，发生模板支撑系统坍塌事故，造成3人死亡、1人重伤，直接经济损失71万元。． 该工程主体是单层混凝土框架结构，长22m，宽12m，高13m，屋面 设计标高13.lm。屋面是现浇钢筋混凝土肋梁楼盖，由主梁 (400mmX1400mm)、次梁(250mmX500mm)和板(1OOmm)组成。标高7.6m 以下部分的立柱、梁，已于9月5日浇筑完成。二次混料室满堂架体的模板支撑系统，由施工员组织人员搭设。2006年9月29日晚开始浇筑二次混料室标高7.6m以上部分。浇筑完柱和梁后，又由北向南 浇筑板。9月3O日凌晨，当板浇筑到一半的时候，施工面突然出现 塌陷，浇筑完的柱、梁和板由北向南全部坍塌，工作面上的施工人 员坠落到地面，被混凝土、脚手架等埋压。 根据事故调查和责任认定，对有关责任方作出以下处理：项目经理、施工员2名责任人移交司法机关依法追究刑事责任；施工单位法定

代表人、木工班长、建设单位副厂长等6名责任人分别受到罚款、记过、警告等行政处分；施工、监理等单位受到罚款、降低资质等级的行政处罚。 二、原因分析 1．直接原因 二次混料室模板支撑系统的刚度和稳定性不合格，是造成这一事故的的直接原因。 (1)搭设存在以下主要问题：一是部分立杆间距过大，超过：《混凝土结构工程施工质量验收规范》中模板体系设计的有关要求；二是同一高度立杆接头过于集中；三是立杆底部底座或垫板不符合规范要求；四是立杆纵横向拉接不符合规范要求；五是没有按规范要求设置纵向和水平剪刀撑；六是整个支撑体系与7.6m以下部分的立柱、梁没有连接。
